Discharge Dispositions and Definitions

When the patient is ready to be discharged from their admission, use the Discharge Encounter conversation to discharge the active encounter. Upon opening the conversation, a Discharge Disposition will need to be selected from the drop-down list. This is used to track the location or status of the patient at discharge. Depending on the patient’s Encounter Type, some dispositions may or may not be selected. See the table below for a list of dispositions and their definitions.

Launch the Discharge Encounter conversation.

Select the appropriate Discharge Disposition from the drop-down.

Enter the Date and Time of the Discharge.

Click Complete.

Encounter Types Associated With Disposition

Admitted to Critical Care or an OR

Admitted into reporting facility as an inpatient to critical care unit or operating room directly from the Emergency department. This option will only appear in the ED Admit conversation.

 

Admitted to an Inpatient Unit

Admitted into reporting facility as an inpatient to another unit of the reporting facility directly from the emergency department. This option will only appear in the ED Admit conversation.

 

Cancelled after Arrival

If the patient arrived for care and the visit/treatment was cancelled but there was documentation attached to it, the disposition of Cancelled After Arrival should be used.

Day Surgery, Inpatient, Minor Surgery

Deceased

Deceased Discharged to Family/Other

Used in the following scenarios:

After a patient has deceased, First Nations usually want to take the body back to the band for burial. The only requirement is that they have a suitable container to transport.

For cases when a body is being transported overseas. They have to meet the standards set by the airlines, but with correct paperwork they can transport the body.

When a deceased patient has been identified to be donated to science for research purposes and the body will go directly to the Body Donation Program.

Discharged Home with Support Services

Support services are available in the home. 

Home could include:  senior’s lodge, attendant care, meals on wheels, homemaking and supportive housing, Chronic Care; Private Dwelling with Home Care, VON.

Discharged Home without Support Services

Patient was discharged home and no support services required.

End of Treatment

End of treatment for a recurring series of treatments.

Used with Cadaver medical service. This disposition will only be used for external cadaver patients.

Involuntary Patient Absconded

Involuntary patient under the BC Mental Health Act absconds from hospital, which can occur while on pass or directly from the unit.

Left Against Medical Advice

Includes patients who left against medical advice (LAMA) and AWOL (absent without leave).

Left AMA After Assessment, No Treatment

No Service Provided/Created in Error

Used when discharging an encounter that was created in error or no longer needed. Also used when no service was provided to patient (patient did not show up and the encounter is not going to be used).

Patient left at his/her own risk following registration. Triage, further assessment by a service provider and treatment did not occur.

Patient left the emergency department at his/her own risk following registration and triage. Further assessment by a service provider and treatment did not occur.

Return to Inpatient Unit

This disposition is used when an Inpatient has been transferred down to the ED for care and is now being sent back up to the Inpatient area. This option will only appear on the ED Admit conversation.

 

Stillbirth

Will only be used on a Stillborn Encounter type.

System Discharge

Will not be part of the picklist.  To be system generated for auto-discharged visits.

 

Transferred to Acute/Rehab/Tertiary MH

Facilities could include another acute, subacute, acute psychiatric, acute rehabilitation, acute cancer center, acute pediatric center, inpatient psychiatry and inpatient chronic/complex continuing care.

Transferred to Assisted Living/Shelters
