
When the patient has been at the sending site for 90 days or more, the Medication Administration Record (MAR) from the sending site should be considered as the BPMH to enter in the Document Meds by Hx screen. Patients coming from other CST Cerner sites may have this information auto-populate.
When the patient has been at the sending site for less than 90 days, the MAR and the verified PharmaNet list of home medications should be combined to create the BPMH.
For medications the patient had been taking at home prior to admission to the sending facility, that were not ordered at the sending facility, the Compliance entry will be on hold with the compliance comment used prior to admission; not ordered at XX Hospital; see MAR.
For new medications that were ordered at the sending facility but had not been used when the patient was at home, the Compliance entry will be taking as prescribed with the comment new order at XX Hospital; see MAR.
For home medications that were continued as active orders at the sending site, the Compliance will be taking as prescribed with the comment continued at XX Hospital; see MAR.
