Printer Selection – Discharge Medication Reconciliation

Where Will My Prescription Print?

It is often required to change printer location for a prescription. On Discharge Med Rec, the printer location can be changed by clicking on the medication, then change the Send To field:

It is important to take the following measures to ensure that prescriptions print at the right place.

PhysicianPhysician

When physician changes printers, subsequent prescriptions default to new printer location for duration of their PowerChart session. Previous prescriptions are still routed to initial printer, and must be manually changed to new printer location.

In the example below, the physician's default printer is Printer A. While prescribing metFORMIN, the physician decides to change the printer to Printer B. The next prescription (pantoprazole) will also be routed to Printer B.

Machine generated alternative text: ¿3 clopidogrel 75mg, P0, qdaily metFORMIN 1 g, PO, BID with food, for 30 day, special instructions, 60 tab, O Refill(s) ¿3 metFORMIN 1g. In the above example, the physician MUST manually change printer selection for clopidogrel to Printer B, to match the printer selection for metFORMIN and pantoprazole.

IMPORTANT: Review all prescriptions to ensure they print at the correct printer.

PharmacistPharmacist

When Pharmacist changes printers, subsequent prescriptions default to the user's default printer. Pharmacist must manually change printer for each prescription. If the pharmacist wants to change printers for subsequent prescriptions, must manually change printer for each prescription.

In the example below, the pharmacist's default printer is also Printer A. While prescribing metFORMIN, the pharmacist decides to change the printer to Printer B. The next prescription (pantoprazole) will still be routed to the pharmacist's default printer, which is Printer A.

Machine generated alternative text: ¿3 clopidogrel 75mg, P0, qdaily metFORMIN 1 g, PO, BID with food, for 30 day, special instructions, 60 tab, O Refill(s) ¿3 metFORMIN 1g. In the above example, the pharmacist MUST manually change printer selection for clopidogrel and pantoprazole to Printer B, to match the printer selection for metFORMIN.

IMPORTANT: If printer location needs to be changed, Pharmacists must change printer location for each prescription.

Planned Discharge Medication ReconciliationPlanned Discharge Medication Reconciliation

The printer that pharmacist selects during Planned Discharge Med Rec will carry over when physician signs Discharge Med Rec.

In the example below, the pharmacist has changed the printer location to Printer C while planning Discharge Med Rec. If the physician signs the Discharge Med Rec without further modifications, the prescriptions will print to Printer C.

Machine generated alternative text: ¿3 clopidogrel 75mg, P0, qdaily metFORMIN 1 g, PO, BID with food, for 30 day, special instructions, 60 tab, O Refill(s) ¿3 metFORMIN 1g. PO, BID with food cj pantoprazole Ditcontinue 40mg, PO, qdaily with food ¿3 pantoprazole Ordered 40mg, P0, qdaily with food G G A Medkations ¿3 (I acetaminophen Ordered 650 mg,PO, as directed Orders After Reconciliation clopidogrel (clopidogrel 75 mg oral tablet) 1 tab, PO, qdaily, 30 tab, ORef ill(s) < Notes for Patient> e metFORMIN (metFORMIN 5GO mg oral tablet) 1g, PO, BID with food, special instructions, 30 day, O Refill(s) < Notes for Patie Reconciliation Status t Meds History O Admission O Discharge + Add ()Manage Plans hQ Reset Reconciliation A Continued Home Medications Orders Prior to Reconciliation ‘ 7 Order Name/Details Istatus o Ordered Di:continu€ Ordered ê e IF ‘ 7 Order Name/Details Status o o o o o o o G o G o G o Printer C o G Printer C pantoprazole 40mg, PO, qdaily with food, 30 day, 0 Refill(s) < Notes for Patient> Prescribed Prescribe Prescribe Printer C

However, if physician makes changes or additions to Discharge Med Rec, new prescriptions will default to the physician's default printer. The physician MUST then check printer selection to ensure all prescriptions print at the same printer.

NOTE: There may be more than one printer in each ward. Pharmacists and physicians must collaborate to ensure that prescriptions print at the same printer.

In the example below, the physician decides to add a Multivitamin prescription to the Discharge Med Rec. This Multivitamin prescription will be routed to the physician's default printer, which is now Printer B.

Machine generated alternative text: + Add ()Manage Plans hQ Reset Reconciliation Reconciliation Status t Meds History O Admission O Discharge Orders Prior to Reconciliation Orders After Reconciliation 7 Order Name/Details Status 7 Order Name/Details Status j A ¿1 Home Medicaflons _____ multivitamin with minerals (Muftivitamin with Minerals oral tablet) o © taL,PO,Çj’ ‘tab, 0ßffßJ< Notesfor Patient> j Printer B Prescribe A Continued Home Medications ______________ ® clopidogrel (clopidogrel 75 mg oral tablet) ( Printer C Prescribed 1 tab, PO, qdaily, 30 tab, 0 Refill(s) < Notes for Patient> ________________ a clopidogrel Ordered 75mg, PO, qdaily _____________ c3 metFORMIN DiEcontinue ® e metFORMIN (metFORMIN 500 mg oral tablet) 1g, PO, BID with faod, for 30 day, special thstwdions, 60 tab, 0 Refill(s) 1g, PO, BID with food, special thstmctions, 30 day, O Refill(s) < Notes for Pat4__Printer C__J Prescribe a metFORMIN Ordered 1 g, PO, BID with food j pantoprazole Discontinue I pantoprazole terC) Prescribe 40mg, P0, qdaily with food 40mg, PO, qdaily with food, 30 day, ORefill(s) < Notes for Patient> a pantoprazole Ordered 40 mn PC) nrlnilv with fririd

The physician must decide whether to change the printer for all prescriptions to Printer B or Printer C. In this example, the physician changes the printer to Printer C:
