In the External History window:
Review the Confidentiality RequirementsConfidentiality Requirements.
Access to a patient’s PharmaNet profile may only occur:
In support of providing patient care (and not more than 5 days before/after a clinical encounter with the patient)
Browsing is not permitted
PharmaNet data must be kept confidential and must not be used or disclosed for any purpose other than to provide care or to provide the patient with a copy of their own profile
The printed or distributed copy of the medication profile must either be maintained on the patient’s medical chart or it must be appropriately destroyed
PharmaNet must not be accessed from outside of Canada
Review the patient demographics.
Click Continue to access the patient's PharmaNet record.

At this point you might see a window requiring you to enter the Patient’s Protected KeywordPatient’s Protected Keyword.
In some cases, the patient will have a Keyword
associated with their medication profile as a privacy measure. If
this happens, there will be a prompt asking for the Keyword.
Ask the patient for it so you can have access to their PharmaNet profile.

NOTES: Patients can request a keyword to protect their PharmaNet profile. They will need to provide this keyword to whomever is accessing their PharmaNet file. In situations where a keyword is needed but the patient is unable to provide (e.g., unconscious), contact PharmaNet by telephone: 1-800-554-0225 (toll free), or 604-682-7120 (Vancouver) . The PharmaNet Help Desk is monitored 24/7 including stats, except on Christmas Day. |
In the event the Keyword cannot be obtained contact the PharmaNet Help Desk (1-800-554-0225) to remove the Keyword. The PharmaNet Help Desk is monitored 24/7 including stats, except on Christmas Day.
Situations where it may be necessary to request that the PharmaNet Help Desk remove the keyword are:
The patient is unconscious or semi-conscious;
The patient is apparently impaired by drugs or alcohol;
There is a reason the patient is incapable of giving consent or is refusing to give consent.
When calling PharmaNet Help Desk to override a patient’s keyword, be prepared to provide the following information:
Site ID (the hospital you’re calling from)
Your name
Problem that you are calling about: PharmaNet keyword removal
Patient’s PHN
Patient’s full name
Patient’s address
Patient’s gender
Afterwards, the patient will be informed by Health Insurance BC (HIBC) that their Keyword was removed, and the patient will be able to re-establish it if they wish to do so.

After accessing the patient's PharmaNet record, the External Rx History window will open.
You will see your patient’s PharmaNet Patient Profile. These are adverse reactions and conditions that community pharmacists have entered.
Update the Adverse Reactions when required. In the images below, you will see two examples. On the left, you will see that nothing has been entered for the patient. On the right, you can see a patient whose PharmaNet Patient Profile displays Adverse Reactions and Conditions.
Adverse reactions may include (but are not limited to) allergies, intolerances, side effects, and contraindications.
Adverse Reactions are not imported into CST Cerner, and so they must be added to the patient’s chart on the Allergy page in CST Cerner as needed. This is done by a nurse during the admission process.

Look for the *ADE in the Conditions section of the patient profile, it denotes an Adverse Drug Event (ADE) record. When you see *ADE, you must look for additional ADE details by matching against the same timestamp in the Adverse Reactions section of the patient’s profile.

PharmaNet FAQ - Troubleshooting Tips
As you compare the External RX History (PharmaNet) with the medications in CST Cerner, you need to know how to read all of the information.
By default, the newest filled medication will appear at the top of the list. Sorting the medications alphabetically will help ensure you do not miss any medications as you compare the two windows.
Click on the Order Name/Details column header in the PharmaNet (External Rx History) window to display the medication alphabetically.

Hover your mouse over the medication listed to see the quantity dispensed and name of prescriber. This medication is already listed in BPMH in CST Cerner.

The Medication Not Reconciled (star burst) icon
indicates that this medication is not yet reflected in the patient’s profile in CST Cerner on the Document Medications by Hx side. After you add a medication to CST Cerner, the Medication Not Reconciled icon will disappear.
It is recommended to always expand bundled medications to view separate dispensing incidences.
In the far left column, the Medication Fill indicator displays a number in brackets. This number indicates that dispensed medications have been grouped or bundled together.
The number indicates the number of fills for the listed medication.
PharmaNet groups medications by separate prescriptions for the same medication name with different forms, strengths, refills, or instructions.
You need to Expand the bundles to view the medication order sentences:

next to a medication.
In this example (Carbidopa/Levodopa), you can see different fill dates for the medication.
In this example (Hydromorphone), you can see different strengths and order sentences for the medication.
Occasionally, there are indicators preceding the medication name with updated statuses such as Discontinued or Adapted-Filled.
When a prescription status is filled, this notation does not appear.
Other Indicator Statuses appear in brackets (e.g. Discontinued, Reversed, Adapted-Filled).
