Request a Best Possible Medication History (BPMH) in the Emergency Department (ED Providers)

At the earliest indication that a patient is likely to be admitted to acute, the Physicians or Nurse should initiate the event that signals the BPMH Team to perform the BPMH in the CST Cerner. This proactive step increases the likelihood that, by the time the consulting physician arrives in ED, the BPMH will be done so that the physician can proceed to Medication Reconciliation at admission without delay.

Go to Emergency Quick Orders, and select ED Perform BPMH from the Consults component.


 

This will insert a capsule icon  into the Events section of the Tracking Shell that BPMH team monitors, to alert them of a new patient who needs to be interviewed.  

When the BPMH team goes to interview the patient, they will remove the capsule icon and insert a comment in the OOD column, BPMH in progress while they are working on the BPMH interview and CST Cerner entry, then change the comment to BPMH done or BPMH complete when it is finished.

Once the BPMH Team has finished documenting the BPMH in CST Cerner, the Orders screen in the patient’s chart will show a green check mark for Meds History, indicating to the admitting providers that the BPMH is complete and they can proceed to Medication Reconciliation at Admission to reconcile orders.

The BPMH Team may initially interview the patients using a paper BPMH form that captures the patient’s PharmaNet profile, and will then input the contents of the paper form into Cerner right after the interview.  They will then put the paper form back into the chartlet. Please DO NOT dispose of the paper from the chartlet. It is important because it provides an audit trail for quality assurance to evaluate how thoroughly and consistently the BPMH is documented.

Some sites may prefer that the BPMH is completed by the BPMH Team when they are back on duty, OR have virtual BPMH coverage for off-hours OR the BPMH shall be taken by the provider admitting the patient after hours.

WARNING: Please do NOT perform a PARTIAL medication history as this will falsely indicate that Admit MedRec has been done, fail to flag anybody else to complete it, and cause problems for Discharge MedRec.
