
Use PharmaNet Integration to Document Best Possible Medication History (BPMH)

PharmaNet data imported by the CST Cerner is stored in the patient’s chart as received to enable the side-by-side view of medications.
As per Pharmaceutical Services Act, personal information means recorded information about an identifiable individual, including business contact information.
Within CST Cerner Millennium, each user is assigned a role profile that is related to the position they hold within the Health Organization. This role profile will give certain privileges that will allow them to access patient information and perform activities related to their role.
Using Role-based Access Control, CST Access & Provisioning Team provides provisioning and de-provisioning for all CST Cerner Millennium users. They work with the users to ensure the appropriate roles are applied.
The Ministry of Health is responsible for all PharmaNet patient profile corrections. If a medication error is discovered in the patient medication profile, the PharmaNet Data Quality Services Team must be advised by secure means (via phone or fax) and provide:
the PHN,
name of the patient, and
a brief description of the discrepancy

Each supervising provider is accountable for PharmaNet access requirements for themselves and all their supervised staff. This includes:
Identifying the most appropriate functional role for each staff member;
Ensuring the role does not provide greater access than what is appropriate;
Adding or removing a user’s access (including temporary replacements);
Changing a user’s access permissions (i.e., a user has changed job roles);
Supervising providers will need to work with CST Access and Provisioning in identifying their supervised staff for access and notifying the team when job roles are changed.
Access with a protective keyword:
Clinicians need to ask for the patient’s protective keyword and consent to access their PharmaNet profile. They also need to inform the patient that consent to use the keyword is also consent to store the data.
The PharmaNet protective keyword must not be stored on paper, and must not be shared with any other person or facility.
If the patient’s protective keyword is not accepted, the clinician needs to contact PharmaNet Help Desk directly (not CST User Support). See below for PharmaNet Help Desk contact info.
Access without a protective keyword:
Situations where this may be necessary are:
The patient is unconscious or semi-conscious;
The patient is apparently impaired by drugs or alcohol;
There is a reason the patient is incapable of giving consent or is refusing to give consent.
In such cases an authorized user must contact the PharmaNet Help Desk:
Phone: 1-800-554-0225 (toll free) 604-682-7120 (Vancouver) to have the patient’s keyword removed. The PharmaNet Help Desk is monitored 24/7 including stats, except on Christmas Day.
The patient will be informed by HIBC that their Keyword was removed, and the patient will be able to re-establish it if they wish to do so.

The Import option through the External Rx History button
After using the Import option, the PharmaNet profile will be permanently saved. This stores the data to enable the side-by-side view screen.
When launching PharmaNet in the CST Cerner, the CST Cerner copies the profile. The copy of information is structured to flow one way.
What does this mean? In the unusual event that a medication is removed from the PharmaNet profile, the CST Cerner will not be updated and the medication will still remain on the CST Cerner profile.
Printing PharmaNet Profile
The PharmaNet profile cannot be printed via the CST Cerner. It is possible to print screenshots of the PharmaNet profile, but this is not recommended because the medication profile may be truncated.
Bundling of Medications
Occasionally, there are indicators preceding the medication name with updated statuses such as Discontinued and Adapted-Filled.
Medications deemed not clinically relevant

Medications not on this list should not be documented unless deemed
clinically relevant on a case by case basis by a healthcare provider
with appropriate scope of practice.
Order Actions available for previously documented medications
Do not use the Cancel/Discontinue action. This has implications on how the information will display in the patient’s discharge summary.
Discrepancies
PharmaNet Reverse-Delete Action: Medications that are entered in error and have been removed from the PharmaNet profile (via reverse-delete action) may appear within the CST Cerner profile with Filled status. Other medication status changes in PharmaNet are updated as expected. To view the most current PharmaNet profile, navigate to the View-Only Medications screen. Always ensure confirmation with the patient.
Same-Day
Duplicate Dispenses: If there are two or more dispenses of the
same drug (DIN) on the same day, only the most recent dispense is
viewable in the PharmaNet profile. To view all dispenses, navigate
to the View-Only Medications screen. Always ensure confirmation with
the patient.
