Document BPMH with PharmaNet using the Scroll Button in Add As Column

To view a patient’s PharmaNet profile, you will access home medications in a similar manner as below:

From the Medication or Orders tab, select the Document Medication by Hx button.   


Or from a Workflow tab, click the Meds History link.



The Document Medication Hx displays Best Possible Medication History (BPMH).

Click the External Rx History drop-down button and select Import.




The PharmaNet External Rx History window opens on the left side, in a side-by-side view, with the Document Medication by Hx window opening on the right.

 

Add Medications to BPMHAdd Medications to BPMH

From External Rx History and Document Medication by Hx windows, you will review a patient’s PharmaNet history. As you interview the patient you will make informed decisions regarding which medications to add to the patient’s BPMH.

WARNING: There is no lock for the Documented Medication Hx screen

CST Cerner allows multiple users to simultaneously view add/or edit a patient’s External Rx History (PharmaNet) and Documented Medication by Hx (BPMH).

There is a potential for duplicate or conflicted entries and will cause errors or omissions downstream.

Ensure only one user edits the patient’s BPMH at any one time.

Clinical areas using a tracking shell are encouraged to update the BPMH status and record events in a timely manner.

 

The Scroll button  in the Add As column is the preferred method for importing patient medications from PharmaNet into the BPMH.

NOTE: It is recommended that you click the black triangle to expand medications BEFORE clicking an Add As button.

 

In the interview, verify the medications as the patient confirms they are taking them.

Click the Scroll button to import a medication from PharmaNet into the BPMH in CST Cerner.

The Scroll button turns orange and the Order Sentences window opens with a list of order sentences associated with the imported product.

Select the most appropriate order sentence and click OK in the Order Sentences window.

Review the order sentence Details.

The Details can be changed if the exact instructions are not available.

Remove extraneous information such as refills, duration, dispense quantity, and/or the Start Date/Time to keep the BPMH tidy.

Enter the dose, route, direction and drug form as appropriate

Document the Patient’s compliance to each medication on the Compliance tab.

NOTE: If no sentences load, click None and manually enter order details

WARNING: Do NOT click Convert Existing Sig as CST Cerner does not correctly perform this function.

 

 

When the Scroll Button does NOT Import a MedicationWhen the Scroll Button does NOT Import a Medication

Medications may not import from PharmaNet for a number of reasons, and CST Cerner will import these as Other Prescription. This is the most obvious sign of a failed import.

In the event that the Scroll button does not import the medication:

Click Cancel in the Order Sentences window.

Document the medication using the +Add button  and search for the medication manually.

You will also use the +Add button  to document any medications that are not in PharmaNet such as supplements, or physician-provided samples for example.

 

 

NOTE: There is a LearningHub e-module to be completed that teaches the specifics of the import process, how to edit and remove already documented medications, and includes an agreement that must be signed in relation to these activities. Refer to your list of recommended and mandatory e-modules for further information.

WARNING: A PharmaNet medication is likely to fail import when the drug name is proceeded by Other prescription or perhaps PharmaCare Nonbenefit.
