
Open PM Office.
Double-click AccessMyHealth Authorized Representative.

Enter details for the Patient and click Search.
Click OK after confirming the details of the patient on the right side.

Enter the Facility Name (SPH St Pauls Hospital).
Click OK.

NOTE: Any facility can be chosen but for consistency, please choose SPH St Pauls Hospital.

The AccessMyHealth Authorized Representative window opens.
Click to highlight the Authorized Representative you want to remove (there may be multiple Authorized Representatives for the patient).

Click on the Remove Person icon.

The Confirm Remove window opens.
Click Yes.

The AccessMyHealth Authorized Representative window WILL NOT show the Authorized Representative you removed.
Click OK for changes to save.

To verify the removal of the Authorized Representative, you must repeat steps 2 to 5 above.
Click OK when you are done.

Do NOT Send Note to AccessMyHealth Patient Portal (Provider)
Do NOT Send Note to AccessMyHealth Patient Portal (Pharmacist)

Review AccessMyHealth Patient Portal Status and Proxy Info in CST Cerner
