
Open PM Office.
Double-click AccessMyHealth Authorized Representative.

Search for the Patient.
Enter the Facility Name (SPH St Pauls Hospital).

Click OK.

NOTE: Any facility can be chosen but for consistency, please choose SPH St Pauls Hospital.

The AccessMyHealth Authorized Representative window opens.

Click to highlight the Authorized Representative that you want to modify
There may be multiple Authorized Representatives for the patient.
Click in the Relationship line (refer to the Red arrow) of the Authorized Representative that you want to modify.
Click the Correct Person icon.

The Authorized Representative – Correct Person window opens.
You can only modify the Email Address and Auth Rep’s Relationship to Person fields.

Click Ok.
If Auth Rep’s Relationship to Person is modified, the following popup window appears.
Click Yes.

Click OK.

The AccessMyHealth Authorized Representative window opens.
Click OK.

To verify the modification, you must repeat steps 2 to 5 above.

Click OK.
Remove a Proxy for AccessMyHealth Patient Portal
Add a Proxy for AccessMyHealth Patient Portal
Do NOT Send Note to AccessMyHealth Patient Portal (Provider)
Do NOT Send Note to AccessMyHealth Patient Portal (Pharmacist)
Send Message to AccessMyHealth Patient Portal Using Message Centre
Review AccessMyHealth Patient Portal Status and Proxy Info in CST Cerner
