
In situations where sharing certain information to the portal may be inappropriate or negatively impact the patient or others, documentation tools exist to support medical staff in documenting sensitive information that should remain confidential and not be shared with the patient or their proxy.

NOTE: This note type should be used infrequently and judiciously, as documentation language and distribution should reflect best practices in patient-centered care. |
In situations where sharing certain information to the portal may be inappropriate or negatively impact the patient or others, a new note type, Provider Note – Do Not Send to Portal, can be used.
The appropriate distributable note should be created for every patient. The Provider Note – Do Not Send to Portal document should only be created in addition to an existing document, and it should only contain information that is not intended for either the patient or their proxy.
The Provider Note – Do Not Send to Portal note type—will be sent to CareConnect and to the Primary Care Provider as well as any selected cc’s via Excelleris, but it will not display in the portal.
To support documentation creation, the note template Do Not Send to Portal Freetext Note is a DynDoc template that will pull the contents of the Provider Note – Do Not Send to Portal component.

To support situations where the contents of the Provider Note – Do Not Send to Portal component are to be inserted into other note types, the AutoText ,,donotsendtoportal can be used.

Use the Provider Note – Do Not Send to Portal when documenting sensitive information that should not be visible to the patient or their proxy in AccessMyHealth. The corresponding note template and AutoText may be inserted into any note type to pull information from the Provider Note – Do Not Send to Portal mPage component.

This note may be distributed to CareConnect, Excelleris, and visible in AccessMyHealth. See Note Type Distribution – Excelleris/CareConnect/AccessMyHealth for more information.
Complete documentation of confidential information to be withheld from AccessMyHealth in the Provider Note – Do Not Send to Portal mPage component.
Click Save.
Scroll to the bottom of the mPage menu and in the Create Note section select More and select the Provider Note – Do Not Send to Portal.

The documentation page will open with the Provider Note – Do Not Send to Portal template pre-populated. Review and/or add sensitive documentation.
Click Sign/Submit to finalize the note or Save to continue editing at a later date.

To finalize the note, in the Sign/Submit note window, add any relevant cc’s.
The Provider Note – Do Not Send to Portal note type—will be sent to CareConnect and to the Primary Care Provider as well as any selected cc’s via Excelleris, but it will not display in the portal.
Click Sign.

Navigate to the Documentation page from the Menu.
Select note type Provider Note – Do Not Send to Portal.
Select note template Do Not Send to Portal Freetext Note.
Click OK.

The documentation page will open with the Provider Note – Do Not Send to Portal template populated.
Review and Sign/Submit as previously described in steps 1-4 above.

NOTES: The following is true of notes created using the ‘Provider Note – Do Not Send to Portal’ note type: Notes will not display in AccessMyHealth. Notes will distribute to CareConnect and to the patient’s GP through Excelleris. |

WARNING Results tagged in the note Provider Note – Do Not Send to Portal may be distributed in error if copied into a distributable note type.
In the example below, documentation from the Provider Note – Do Not Send to Portal has been copied and inserted into a Patient Discharge Summary. The Patient Discharge Summary is a distributable note type, and the tag reference will be available to view at the bottom of the document in AccessMyHealth. |
