Add a Proxy

The Release of Information (ROI) Clerk receives the Proxy Access Request form through email or from the Task Queue in the Proxy Management System.

Open PM Office.

Double-click the AccessMyHealth Authorized Representative conversation.

Enter details for the Patient and click Search.

After confirming the details of the patient on the right side, click OK.

NOTE: If the patient record is not found in CST Cerner, please verify with the requestor that the submitted patient information is accurate. If it's confirmed that the patient truly does not exist in the system, proceed to reject the proxy request.

 

The Organization window opens.

Search the Facility Name (SPH St Pauls Hospital).

Click OK.

Click the New Person icon under Authorized Representative Organizer to start linking an authorized representative.

Click on Search for Auth Rep.

For Relationship Type, choose Authorized Representative.

For Auth Rep’s Relationship to Person, choose an appropriate relationship from the drop-down.

Enter the Authorized Representative’s email address.

Click OK.

Click Yes.
